Are in intensive care or high dependency units and are due to return home, facing a long-term recovery
Have physical or learning disabilities and require long-term support in their home
TRANSITIONS

Our Transitions service enables us to respond quickly to your needs inside and outside the home.
Transitions is a planned service offered to those who already receive support from us and includes:

Transport services such as hospital to home transfers. We can arrange for a Support Worker to meet you at the hospital with suitable transport. We can also provide transport to visit your family, the shops or social events

We can prepare your home for special events, discharge from hospital or if you've been away on a trip (ensure heating, electricity and water are working, there’s a well-stocked fridge and hot meals are prepared)

We can arrange regular food or meal deliveries, install home adaptations such as grab rails, key safes, plumbing and electrics

We can provide vetted and reliable contractors for repairs, home maintenance, gardening services and cleaning
